Three violation listed as "Serious" and one as "Other"

When government safety organization inspect an aluminium plant, a majority of the time they can find a violation if they look hard enough. Here is a follow-up to a company that we posted earlier that had been inspected frequently by OSHA:

OSHA is fining the Aluminum Company for health and safety violations.
The fines total exceed $60,000. According to information provided on OSHA's website:

In early July 2017, the Aluminum Company was issued four OSHA Safety violations exceeding a penalty of $25,000. Three are "Serious" and one is listed as "Other." These violations were opened in May 2017 of this year and they include Safety Requirements for Scaffolding, Powered Industrial Trucks, General Requirements for all Machines, and General Requirements.

In July 2017, the Aluminum Company was issued four additional OSHA Health violations totaling a penalty exceeding $32,000. Those violations were opened at the end May 2017, and include Respiratory Protection, Permit-Required for Confined Spaces and Hazard Communication. All four violations are listed as "Serious."

According to the United States Department of Labor's website, these are the Aluminum Company's first penalties.

In early 2016, the Aluminum Company had Informal Settlements exceeding $13,000 in penalties. This included 'Control of Hazardous Energy (Lockout/Tagout)' and 'General Requirements for all Machines'.

The company had further Informal Settlements in 2015, 2014 and 2013 all exceeding $20,000 in penalties. OSHA's website did not state whether the company paid those prior penalties.

The Aluminium Plant Safety Blog has chosen to omit the company name and location because the company does have the opportunity to argue against the fines.
This aluminium company was cited for failure to adhere to the following:

  2. Powered Industrial Trucks (link),
  3. General Requirements for all Machines (link),
  4. General Requirements (link),
