Sadly there have been several recent fatalities that we have yet to post. In our experience almost all incidents involve a process or tasks that the worker had done before countless times safely. We argue that the task or procedure was always hazardous. It was by shear luck that an incident did not occur before. We feel the task in this incident was done countless times before without an incident too. Till one occurred. We are grateful no one was injured. Here is the story:

Police in Canada are investigating an extremely dangerous and unsafe incident
in Oakville this afternoon (May 20, 2020) in which countless pieces of aluminum
came flying out the back of a transport truck and into live traffic.
The
incident occurred at Dundas Street East and Prince Michael Drive in Oakville
just after 1 p.m.
Halton
police Constable said in a video from the scene the dolly wasn't properly
secured in the trailer and the aluminum pushed forward during a stop-and-go and
"snapped the hinges clean-off," causing the load to spill onto the
roadway. Police have closed
multiple lanes and said the truck driver is facing Highway Traffic Act charges.
Town of Oakville road crews are on scene to help with the clean-up. Thankfully,
police said, no one was injured.
We are thankful that no one was injured or killed. We have posted
incidents where raw material, finished product, etc. has fallen off a truck and
injured or killed and a pedestrian or motorist.
No doubt in our mind that this task was done before countless times.
The load should have been secured in such a way that the carts could not move.
That was not done. It does not matter the distance all loads should be properly
secured. Including inside the factory gates. All too often we will observe
material being transported inside the plant and no load securement is done. Does
