How do you hear about the incidents that get posted?

One question the APSB gets asked occasionally is "How do you hear about the incidents that get posted?". Here's the answer(s).

The APSB does not post based on rumor or word of mouth.  In all honesty, that decision was made from a practical standpoint (not enough time). There are far more incidents that occur in our industry then what is posted on this blog.   

The aluminium plant safety blog post incidents that are written about in the media (newspaper, radio, tv, etc.). 

Our hope is that our posts maybe used to prevent future incidents from occurring. Understandably, some of the incidents lack enough information to fully understand what the root cause was. Nevertheless, the APSB believes it is easier teaching workers about safety when they are provided with examples of incidents of what could go wrong.
