"the cause of death was ruled accidental....."

Here is a recent fatal incident.

The (name of US State) Occupational Safety and Health Administration (IOSHA) has confirmed it has opened an investigation into a death earlier this month at an aluminium factory.

No foul play was suspected in the death of a worker according to a press release issued late one day during the week of June 23, 2019 from local Police Department Captain.

Police were called to the scene in early June 2019 to an aluminium company. The worker was found to be already deceased when police arrived.

The local Detective worked with the County Coroner to investigate the death. Drugs and alcohol were not considered a factor, according to a statement issued late one day June 23, 2019 by the local police captain.

A local newspaper request for additional information from the Occupational Safety and Health Administration (OSHA) was referred to OSHA, and was answered late one day.

“We can confirm that OSHA did receive a report of a workplace fatality at the facility,” stated a person with the safety organizaion. “OSHA has opened an inspection.”

The employee had worked at the aluminium company as a furnace operator for the past nine months prior to his death.

A request sent to aluminium company requesting additional information has not been returned as of press time.

UPDATE
In June 2019, a local police were called to an aluminium company in the USA. Officers on scene, located a worker deceased. Local Police Detective in cooperation with local County Coroner investigated the death. Upon the completion of the investigation the cause of death was ruled accidental. There was no foul play suspected in this case.
