"One dead and one injured when grenade (cylinder) explodes"

The presence of pressurized cylinders in our workplace is common. The transportation both short distances (within the workplace) and long distances (from outside to inside the workplace) is a hazard that many workers fail to realize the seriousness 

One dead and one injured when grenade explodes in aluminum business

The cylinder exploded when it fell inside the business located in the (name omitted) neighborhood

A grenade exploded inside an aluminum business that left one man dead and another injured in the (name omitted) neighborhood, in the metropolitan area of (name omitted).

The event was reported to the authorities around 01:00 in the afternoon in a business, located on the (name omitted) highway at the height of the aforementioned settlement.

According to the police report, a cylinder exploded at the scene and a "person shot due to the impact" came out.

According to information from the authorities, it was a grenade that they had with lead and that exploded when it fell.

At the scene, a man was injured, so he was helped by Civil Protection personnel and transferred to a hospital for medical attention.

The area was delimited and guarded by the State Police, while the personnel of the State Prosecutor's Office and the Ministerial Police carried out the legal proceedings.

The body was transferred to the facilities of the Forensic Medical Service for the corresponding legal procedures.

We offer our sincere condolences to the deceased worker’s family, friends, and coworkers. We pray that the injured worker recovers fully from their injuries.

We pray that this incident will be used to prevent recurrence. We hope that this incident is used at workplaces throughout our industry to emphasize the hazards of pressurized cylinders in our plants.

We have sadly reported on numerous similar incidents where unsecured pressurized cylinders have fallen over and either ruptured like a “grenade” in this incident or have been launched like a rocket. In the past incidents workers have been injured or killed.

Please note the word grenade has been used in similar incident news media articles. Because in many ways the damage that occurs when a pressurized cylinder explodes is like dropping a grenade on a work floor.
