"man crushed by a sliding door dies...."

Hidden hazards are present in every workplace. They only are realized when an incident occurs. It is our sincere hope that by publicizing incidents that we can contribute to making the reader’s plant(s) safer. Here is a recent incident.

A worker died one night in a workplace accident at the facilities of the (aluminum) company. The victim is a 50-year-old man, an employee of the firm itself. The accident occurred at around 10.30 p.m. at the facilities of (aluminum company). The plant is one of the production sites of a family business dedicated to the extrusion and lacquering of aluminium. The worker, (name withheld), had been part of the company for almost a decade and shared a flat in (town name omitted) with another operator from the same company. He was single and had family in (town name omitted), from where he has moved to (Providence name omitted) after hearing the tragic news.

The Civil Guard is investigating what happened to clarify the circumstances of the workplace accident, which is the fifth fatal workplace accident to occur in province name omitted this year. As reported by (police department), the accident, apparently, occurred when the worker was "trapped by the horizontal sliding door of the facilities, which fell after leaving its guide, probably due to the absence of the piece that blocks its movement" and the victim "died by crushing". The union deeply regrets the new death of a worker who, "once again, could have been avoided", and trusts that the investigation can clarify "the causes and responsibilities".

We offer our sincere condolences to the deceased worker’s family, friends, and coworkers. We are at a loss for words to describe this tragedy. As it appears the worker was in the wrong area, at the precise second the gate failed. We have a video of a gate, which appears to be one of the truck gates to the street. We will confirm that it is the gate and update this post with a photo. We do question if there was a man gate or workers were forced to walk through the truck gate.

Lastly, due to our travels throughout the globe we were taken aback that the deceased worker resided in a small town that we had stayed in.
