The failure to follow training injures and kills workers. Here is a recent incident where a worker’s failure to follow their training cost them their life.
His name was (name omitted),
the worker who died around 11.30 a.m. in an accident at work in the industrial
center of (location name omitted). The man would have turned 49 in August. He
leaves behind his wife and a daughter.
According to an initial reconstruction, still being examined by the investigators, (the deceased worker) would have been crushed by the forks of a forklift inside the plant, against a large aluminum coil, after getting out of the vehicle. The death would have occurred while the worker was writing some initials with a marker on cellophane. The firefighters worked for a long time to free the body, which was stuck.
The helicopter doctors, the fire brigade, the carabinieri of the (local) station and the inspectors of the (government safety organization) intervened on the spot. The dynamics have not yet been precisely defined. The investigations also concern the forklift involved. After the surveys and the hearing of work colleagues, the body was transferred to the morgue of the hospital in (town name omitted). The autopsy will be performed as ordered by the judicial authority.
We offer our sincere condolences to the deceased worker’s family, friends, and coworkers. It appears the forklift operator got out of the forklift to write something on an aluminum roll and failed to engage the brake. The forklift continued to move and pinned the operator between the forks and an aluminium roll.
We are confident that the operator was instructed that prior to exiting the forklift, the forks must be lowered to the ground, and the vehicle is turned off. The forks on the ground would minimize the chance of the forklift moving.
